Minutes PPG meeting Wednesday 4th July 2019 12:30 to 14:00

1) Present: Marilyn Miles Chair, Sally Millett secretary, Stuart King, Suzi Colley Practice Manager, Richard Easthope, Betty and Paul Young. Stuart King was welcomed as a new member. He was previously part of Locking Hill PPG in Stroud.

2) Apologies for Absence: Pat Fitch, Mike Kelly, Dr. Meikle, Karon Arnold (Practice Pharmacist), Jo Hofman who has left the PPG.

3) Minutes of last meeting: Wednesday 27th February – the date of the next meeting was changed from July 5th to Wednesday 4th July. These were then accepted as correct.

4) Matters arising not covered by the agenda: The Patient Participation Group is no longer part of the Nailsworth Health Partnership as this umbrella is no long needed. PPG is a stand along partnership and is covered by the Practice insurance.

5) Blood Donor sessions: Nailsworth Town Council has agreed to hire the Town Hall to the blood donor service now that they can no longer use the Forest Green Rovers site.

6) PPG Cluster meetings Next one is Thursday 1st August 18:00 – 20:00 at Stroud Fire Station.

7) PPG County Meetings Friday mornings at Churchdown Community Centre 19th July and 18th October. The link is www.gloucestershireccg.nhs.uk.

8) Primary Care Networks. There has been a new configuration of GP practices to replace the clusters that we had before. This is a national initiative and has been set by NHS England – they will be called Primary Care Networks (PCN).

Price’s Mill Surgery is part of the Severn Health PCN. The hope is these different PCNs consisting of GP practices will work better together with hospitals and other health services. We have seven GP practices in our network. With the previous “cluster” there was a Dementia Pilot run at the Minchinhampton, Chalford, Painswick and Price’s Mill surgeries. A trained dementia nurse held surgeries for patients. These pilots no longer continue but many of the results have been rolled out across the area.

a) There is now a 10-year long term plan for the NHS and within this is a 5-year plan for General Practice which is now at the heart of development. 80% patient contact 20% funding for community services into hospitals or services coming out into the community. The plan is for health organisations to work more closely together instead of in competition. Current GP funding is £156 per patient with extra for those who need specialist service. There will be a provision of services at a network level e.g. social prescribers, clinical pharmacists, physiotherapist and paramedics over several years. There will be extra funding for surgeries who provide specialist care and services.

b) Social Care General Budget. Bids have been blocked in the past because of a lack of funding. The AIM is evolving towards an integrated care system. One budget for community care to be jointly managed a number of integrated locality partnerships, Acute, Mental Health, GPs and Practice Managers. The eventual aim is to extend it to County Level but there is no shared budget at the moment.

c) NHS England has announced all practices need to be in an integrated partnership. General practice will need to change. Care will be more personalised. In terms of the PCNs NHS England has said they were to be 30-50,000 patients in size as these have proven to work best. In determining which PCN Price’s Mill would be part of, it was important to be part of a group that had similar values to Price’s Mill. National purpose is to improve outcomes for patients – common practice across the system allows access to patients records.

d) Social prescribing in this area it is run by Independence trust. Not proved as efficient as hoped for so this is being looked at. They tend to help with Social needs -signposting to support groups etc -needs to be local and connected to GP knowledge.

Options social prescribing is Commissioned at County level and is being reconfigured direct to individual surgeries. See appendix for who is working in our PCH – Severn Health

e) Clinical Pharmacist. Stroud locality has improved the overall services. There is a need for a use of funding to get more clinical grade pharmacists who would release GPs time for the work they are qualified to do i.e. checking medicine use could be given to pharmacists. The name Cluster has been replaced by Primary Care Network.

9) Roll out of National Diabetes Prevention Programme. This is now in place across the county.

10) Dementia Friendly GP Practice: There is to be a new 6-month dementia pilot across the Stroud district which aims to reduce the number of avoidable admissions to hospital for people living with dementia. This is in collaboration with Alzheimer’s society and will have 3 “phases”- early stages after diagnosis, mid stage and end of life care. Rachel Peak from Dementia Training and Education Strategy (DTES) has been seconded to this project starting on 1st July.

11) Pharmacy Feedback. There was discussion around the results to the survey carried out and Karon Arnold has asked to share this with the area manager for Lloyds before asking the local manager to come to a future PPG.

12) Feedback from the PPG stand at the town. One person asked for General first Aid course. Suzi Colley said the practice would be willing to offer a room if it were free. There has been a request for a different chair to be provided for the blood pressure area in the lobby. It was felt it should be adjustable to help give accurate results. Suzi Colley would look into this.

13) Date of next meeting. Thursday 17th October. Prices Mill 12:30 to 14:00.

Appendix 1. Community well-being service – Stroud and Berkeley Vale.

Each locality will have a Social Prescriber Facilitator and a Community Well-being agent. This will be supported by a group pf volunteers who have either been through the service themselves or are form the local communities.
