Registering With The Practice
We provide care to 9,000 patients in the Nailsworth area. The practice provides a full range of general medicine, maternity, contraceptive and children’s services. Further information can be found on the pages of this website and it is summarised in our Practice Leaflet that can be downloaded here.
To register with the practice, you must live within our practice area, and be eligible for NHS treatment. The receptionist will confirm this for you. You can also check if you live within our practice area for yourself by entering your postcode below.
Please bring in your NHS card, or ask the receptionist for a form. We ask all patients to provide proof of ID and proof of address, and to complete a new patient questionnaire before we register them – this is to gather basic information about you.
If you wish to pre-register please use our Pre-registration Form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. When you register you will also be asked to fill out a Medical Questionnaire. This is because it can take a considerable time for us to receive your medical records. If you fill this in on-line, you can sign this form to confirm that the details are correct next time you come to the surgery.
